JOB DESCRIPTION
The Community Health Centre of Buffalo is now looking for an experienced Family Planning Nurse Practitioner to provide outpatient care to patients in our cutting-edge facilities. Duties:
Follows all of CHCB’s clinical policies and procedures, including infection control principles and confidentiality.
Physical and psychosocial health state are assessed using a medical history and physical examination.
Order and interpret laboratory testing.
Within the speciality area of practice, disease and physical ailments are triaged, diagnosed, and treated.
Provides therapeutic and remedial actions, including medication prescribing, as well as preventative health maintenance and education.
Educates and counsels patients, families, and groups.
Participates in quality-improvement activities.
Complies with all guidelines established by the collaborating physician in compliance with all regulatory agencies that control health care delivery.
Performs specific assignments as needed.
Prepares written documentation as required by the profession and the department; manages, distributes, analyses, and safeguards information for required records, reports, and statistics as ordered.
Other relevant duties are performed as assigned or required.
Minimum qualifications:
Licensed in New York State, board certified, and a Registered Professional Nurse Practitioner with 5 – 7 years of experience providing family planning services, including Long Acting Reversible Contraception.
Must be computer literate, with knowledge of MS Word, Excel, and Outlook. Experience with eClinical works is preferred.
Salary range: $103k to $108k.
