Highly Specialist Clinical Pharmacist – Gastroenterology

If you’re a motivated, forward-thinking pharmacist looking to make a real difference to patients with complex gastrointestinal conditions, this role offers an exciting opportunity to work at the heart of a specialist, multidisciplinary service. You’ll be joining a supportive, innovative pharmacy team where your expertise can influence clinical decisions, shape service development, and improve outcomes for patients with inflammatory bowel disease, liver disease and other GI disorders.

About the Role

As the Highly Specialist Clinical Pharmacist for Gastroenterology, you will be responsible for delivering advanced, patient-centred pharmacy services across both inpatient and outpatient settings. Your clinical contribution will support pharmacist-led clinics, biologic infusion services, therapeutic drug monitoring and medicines optimisation for patients on complex, high-cost treatments.

Working closely with consultants, nurses and the wider MDT, you will:

  • Provide independent prescribing (or work toward this) for biologics and other complex therapies.
  • Offer expert medicines advice, including monitoring, interactions, safety and adherence support.
  • Lead on implementing NICE guidance, support pathway development, and ensure treatments align with commissioning policies.
  • Participate in ward rounds, infusion clinics and service evaluation to reduce harm and optimise patient outcomes.
  • Contribute to specialised audits, formulary submissions and cost-effectiveness reviews for high-cost medicines.

This role also offers opportunities to support clinical trials, develop treatment guidelines, mentor pharmacy staff and influence practice through quality improvement and research.

What You’ll Be Doing

Clinical Practice

  • Run or support gastroenterology clinics, offering tailored prescribing and treatment optimisation.
  • Lead therapeutic drug monitoring and provide medicines reconciliation for complex and high-risk patients.
  • Deliver specialist pharmaceutical care to patients with IBD, liver disease and other GI conditions.
  • Ensure ongoing compliance with NICE recommendations and local commissioning arrangements.

Service Leadership & Development

  • Support the development and alignment of prescribing guidelines and treatment pathways.
  • Analyse drug expenditure, prepare business cases and contribute to commissioning submissions.
  • Lead and participate in QI projects, audits and service redesign to drive continuous improvement.
  • Provide specialist advice to consultants, GPs, community teams and other stakeholders to ensure seamless, safe transitions of care.

Education & Training

  • Mentor junior pharmacists, technicians and students across the gastroenterology service.
  • Deliver training for prescribers, nurses and MDT colleagues, contributing to governance and risk-management processes.

Person Specification

Essential

  • GPhC-registered pharmacist with a postgraduate diploma in clinical pharmacy (or equivalent experience).
  • Significant hospital pharmacy experience, ideally with advanced practice exposure.
  • Ability to interpret complex clinical information and optimise treatment for patients with comorbidities.
  • Strong understanding of NICE guidance, high-cost medicines, Blueteq and commissioning pathways.
  • Proven involvement in audits, research or service improvement.
  • Excellent communication, leadership and mentorship skills.

Desirable

  • MSc or advanced postgraduate qualification in clinical pharmacy or gastroenterology.
  • Experience in GI, hepatology, infusion therapy or biologic services.
  • Independent prescriber qualification (or commitment to complete within 12–18 months).
  • Experience developing guidelines, managing QI projects or contributing to cross-site service development.
  • Previous line management or clinical supervision responsibilities.
