We presently have openings for experienced and motivated Social Workers in the Adult Social Care Hospital Team who can respond to general inquiries and crisis circumstances.
In our Adult Social Care Hospital Team, we help our most vulnerable residents get their lives back on track by taking a strength-based approach and placing the person at the centre of all we do.

Our Adult Social Care service consists of highly professional and helpful multidisciplinary teams. We have a “can do” and “no wrong door” philosophy. Our social care teams collaborate and go above and beyond to ensure that our residents receive the care they require.
We collaborate closely with health and other partners to ensure that people do not travel to the hospital when they do not need to, and that if they do, they are only there when they are needed and may return home as soon and safely as possible.
We have a variety of intermediate care and discharge assessment programmes that focus on keeping people in their own homes.
We have regular multidisciplinary forums, which allows our residents to easily access health and social care professionals that communicate with one another in order to give holistic support when needed.
We understand that working in Adult Social Care can be stressful, so we provide our employees with adequate support. You will have regular supervision and access to training. We place a major emphasis on helping our employees learn, grow, and develop in order to reach their full potential. We listen to our social care professionals about the best method to deliver our services and welcome criticism and recommendations on how we can work together better.

Manage a caseload by properly planning and prioritising the workload while receiving regular supervision and coaching.
Working as part of a multi-disciplinary team, linking people and their families to their community and social networks, allowing them to live within their own communities,
To collaborate with interdisciplinary colleagues to ensure that individuals receive the right care at the right time and in the right location.
To use a strengths-based approach to help individuals live the best lives they can while reducing need on long-term care.
To help deliver and sustain high standards of performance across the service.
To work within the resources allotted in our delegation structure.
Take an evidence-based approach to practice
Take an active role in your own supervision and appraisal, in accordance with the departmental supervision policy and the council appraisal policy.
Accept responsibility for your own education, including mandated and suggested training.
To lead certain areas of work as needed.

What are we looking for?
We are looking for qualified social workers who are registered with Social Work England. You will have at least 2 years of post-qualification experience in statutory social services, typically in a local authority environment. You must have a thorough understanding of best practices and applicable legislation. You will be motivated with a professional and “can do” attitude.
Interview date
Interviews will be held on July 29th & 30th, 2024.
Interviews will be held in person, allowing candidates to meet the team and gain firsthand experience with our workplace. Furthermore, this strategy enables the recruiting manager and team to make contacts with potential team members.
