JOB DESCRIPTION

 

The Hospital Discharge Team at North Devon District Hospital is made up of experienced social workers, social care assessors, nurses, paramedics, and coordinators who collaborate with hospital staff to provide the best discharge alternatives for patients we interact with.

The social care assessor helps clients explore their alternatives for care and assistance after discharge from hospital.This entails examining care and support requirements to determine which of the discharge pathways is most suited.

A thorough understanding of the Mental Capacity Act is required for this position, and prior experience doing mental capacity examinations would be advantageous.

Once a person has been discharged from the hospital, the team is responsible for completing Care Act Assessments for individuals to determine their ongoing needs for care and support.

As a social care assessor, you will play an important role in ensuring that the people we work with have the resources they need to succeed.
