You will establish an effective working relationship with young people who are eligible to leave care services in order to motivate and effectively support them in pursuing education, training, and employment, as well as ensuring they manage their finances effectively, live in appropriate housing, and possess the necessary emotional resilience and independence skills.

The Leaving Care staff is a small but committed staff that helps us deliver the best possible support to our vulnerable young people and care leavers.
The successful candidate will manage and support an assigned caseload of young people aged 16 and up who are eligible to leave care services, with a primary responsibility for ensuring the provision and coordination of practical, personal, emotional, social, and financial advice to enable care leavers to make a successful transition to adulthood and promote independence.
You will receive support to perform the following duties, responsibilities, and training.
- Conduct thorough examinations of young individuals leaving care.
- To serve as a Personal Advisor for a caseload of young people leaving care.
- To promote the Leaving Care Service to partner agencies.
- Work in collaboration with other specialists.
- To aggressively refer care leavers to appropriate services.
- Maintain adequate case management records.
- Work flexibly to address the needs of young people leaving care. This could mean travelling out of the neighbourhood and working in the evening.

You must be organised, adaptable, IT literate, and ready to work as part of a team.
