Job Description
We are looking for an enthusiastic individual to lead the Transition Service for children with complex needs moving into adult services.
The Transition Nurse’s primary role is to enhance the transition experience for young people and their families, providing expertise in adolescent care while developing services for those with complex long-term musculoskeletal and spinal conditions.
Candidates should have prior hospital nursing experience with children and young people, as well as experience in a transitional role for those moving into adult services.

The postholder will primarily be based in the hospital, providing direct specialist care, education, and support for transitional care patients, and will act as a resource for optimizing available services. Responsibilities include:
– Delivering care in line with national and Trust policies, conducting needs assessments, and continuously developing the service for patients and their families during their transition.
– Offering independent support to patients and families, preparing them for the transition process.
– Supporting young people aged 16 and above when placed on adult wards within the Trust.

The postholder will lead all aspects of the transitional service, focusing on empowering and educating patients and their families throughout the transition to adult care. This role aligns with the Trust’s values and performance standards, ensuring effective transitions from Children and Young People Services to Adult Services.
Key responsibilities include:
– Providing a clinical-led transitional service for older children and young adults with complex conditions.
– Demonstrating considerable expertise in the care and management of patients transitioning from children’s to adult services, and advising multidisciplinary teams.
– Facilitating high-quality, individualized care for patients and their families, working collaboratively with the healthcare team.
– Acting as an autonomous practitioner responsible for ongoing clinical management in various settings.
– Advocating for families and providing expert support based on best practices.
– Maintaining a flexible approach to clinical support and fostering a safe environment for the wellbeing of staff, patients, and families.
– Staying updated on the transition agenda with NHSE.
– Serving as a clinical role model and leader in transitional care for both local and wider NHS staff.
