Healthcare Assistant – Erewash Care Team

We are seeking a dedicated and compassionate individual to join our specialized end-of-life care team. As part of this role, you will become a valued member of a large and experienced group of Healthcare Assistants, overseen by a District Nurse. This team is part of a broader Integrated Community Team, which includes Community Nursing and Community Therapy services.

Shifts are structured as early and late, each lasting five hours. Early shifts are from 08:00 to 13:00, and late shifts run from 16:00 to 21:00. Your working hours will be allocated over a 7-day period, with one in three weekends included. Each shift will typically have a team of four staff members, allowing for both collaborative joint visits and independent visits within the community.

Our team provides end-of-life care for patients in their homes across the Erewash area. This means you will be required to drive between visits and our two main bases: Ilkeston Community Hospital and Long Eaton Health Centre.

We are looking for someone with a genuine passion for providing high-quality, compassionate end-of-life care. Previous healthcare experience is important to ensure that you are equipped with the necessary skills to perform this role effectively.

Key Duties:

Provide personal care to end-of-life patients in their homes.

Assist with manual handling and use equipment provided in the home.

Manage continence care, including catheter management.

Offer pressure area care.

Administer medication in accordance with the Medication Administration Record (MAR) sheet.

Communicate with all staff members in the Integrated Community Team and utilize a laptop to maintain patient records on Systmone.
