JOB DESCRIPTION

 

We are looking for skilled and highly motivated persons for the position of Mental Health Support Worker. These are interesting and active opportunities within the CRISS (Crisis Resolution Intensive Support programme), a dynamic and adaptable programme that provides 24/7 crisis assessments and home-based treatment to working-age people around the city. The service is now being developed to bring together both sides of the service in various locations throughout the city while adhering to Core Fidelity criteria.

We are looking for Mental Health Support Workers that have previous experience working in acute care and are eager to help our service users achieve better mental health in the community. The best candidates would have experience in mental health, supporting and addressing the diverse individual physical, mental, and holistic requirements of service users while working under the direction and supervision of senior colleagues.

You will work as part of an MDT-led team whose goal is to prevent hospital admissions and encourage early discharge through home-based care. You will be expected to collaborate with service users, carers, and the Multi-Disciplinary team to provide care for people with acute mental health issues.

Applicants must hold a full driver’s licence and have access to a vehicle for work purposes.

The successful applicant will be part of a multidisciplinary team that collaborates closely with other community and inpatient mental health providers. As a Mental Health Support worker, you will be responsible for a wide range of care delivery and support tasks, as well as providing a high degree of care to service users in the community. This will include providing care in accordance with agreed-upon care plans, at times under the supervision of a qualified practitioner. There will be opportunities to work independently with service users, and you will receive regular supervision to assist you in this job.

Applicants should have some experience working with a variety of clients with complex mental health issues, as well as the capacity to respond effectively in crisis circumstances and appreciate the importance of appropriately recording/reporting such instances to senior team members. You should also have a thorough awareness of how a mental illness can affect an individual’s well-being, as well as a strong dedication to the recovery model and its implementation in care delivery.

Participation in the provision of planned care.
To promote social inclusion for service users, facilitate and support access to a variety of community possibilities such as work, social groups, education, spiritual pursuits, and leisure activities in order to maintain overall well-being.
To work independently with service users in their homes as needed, which may involve home visits or other community-based activities.
Participate in CPA meetings as required.
Assist individuals in identifying their own attainable and meaningful recovery goals, and collaborate to develop recovery objectives in accordance with the service care route and the supervision of senior team members.
