About the Role

We are seeking an experienced and compassionate Senior Community Mental Health Practitioner to join our Community Mental Health Team (CMHT). This is a key clinical role, supporting individuals living with complex and enduring mental health conditions while helping to shape high-quality, recovery-focused care within the community.

As a senior practitioner, you will take responsibility for coordinating care for a complex caseload and will play an important leadership role by supervising Band 5 Community Practitioners and Band 3 Community Healthcare Support Workers where appropriate. Working closely with the Deputy Team Manager and Team Manager, you will ensure care planning and risk management are personalised, collaborative, and focused on recovery.

Key Responsibilities

  • Manage and coordinate care for a complex caseload of adults with severe and enduring mental health needs
  • Complete comprehensive mental health and risk assessments when clinically appropriate
  • Develop, review, and adapt care and risk management plans in partnership with service users
  • Deliver evidence-based, recovery-focused therapeutic interventions aligned with agreed care plans
  • Promote choice, independence, and personalisation, recognising service users as experts in their own care
  • Use recovery tools such as Wellness Recovery Action Plans (WRAP) and Recovery Star to support empowerment
  • Ensure service users receive appropriate interventions outlined in their care plans, including physical health monitoring
  • Work proactively with carers and families, recognising their vital role in recovery
  • Monitor changes in mental health needs and respond promptly, adjusting care plans as required
  • Participate fully in clinical supervision, caseload reviews, and service development activity

Qualifications and Experience

Essential

  • Registered Mental Health Nurse or recognised Mental Health Practitioner
  • Recognised mentorship qualification
  • Current UK professional registration

Desirable

  • Experience working within a community mental health setting
  • Leadership training or qualification
