Job Description

John is at home and in his local community, receiving the right help at the right time and in the right location. He has a fulfilling life and is content at home thanks to the assistance provided.

In our Adult Social Care Hub, we help our most vulnerable residents get their lives back on track by taking a strength-based approach and placing the person at the centre of everything we do.

This is why our Social Workers have the opportunity to get to know everyone they work with. Simply said, we help people improve their lives and become as independent as possible.

We place a major emphasis on collaborating with our partners to provide complete and seamless support when individuals need it.

We are proud of the care and services we provide to the local community.

Because we are Bracknell Forest.

We presently have openings for experienced and motivated Social Workers at the Adult Social Care Hub who can react to general inquiries and crisis circumstances.

What is it like to work in our team?

Our adult social care service is made up of highly professional and helpful multidisciplinary teams. We have a “can do” and “no wrong door” philosophy. Our social care teams collaborate and go above and beyond to ensure that our residents receive the care they require.

We collaborate closely with health and other partners to ensure that people do not travel to the hospital when they do not need to, and that if they do, they are only there when they are needed and may return home as soon and safely as possible.

Our Early Intervention and Rehabilitation services aim to keep people in their homes.

We have regular multidisciplinary forums, which allows our residents to easily access health and social care professionals that communicate with one another in order to give holistic support when needed.

We understand that working in Adult Social Care can be stressful, so we provide our employees with adequate support. You will have regular supervision and access to training. We place a major emphasis on helping our employees learn, grow, and develop in order to reach their full potential. We listen to our social care professionals about the best method to deliver our services and welcome criticism and recommendations on how we can work together better.

You will receive support to perform the following duties, responsibilities, and training.

  • Manage a caseload by properly planning and prioritising the workload while receiving regular supervision and coaching.
  • To work as part of a multidisciplinary team, linking people and their families to their community and social networks, allowing them to live in their own neighbourhoods.
  • Collaborate with interdisciplinary colleagues to ensure that patients receive the correct care at the right time and location.
  • To use a strengths-based approach to help individuals live the best lives they can while reducing need on long-term care.
  • To assist in delivering and maintaining high levels of performance throughout the service.
  • To work within the resources allotted in our delegation structure.
  • To practise using evidence-based methods.
  • Take an active role in your own supervision and appraisal, in accordance with the departmental supervision policy and the council appraisal policy.
  • Accept responsibility for your own education, including mandated and suggested training.
  • To lead certain areas of work as needed.

You will receive the following benefits:

  • An inclusive and social work atmosphere.
  • We provide a hybrid working paradigm to encourage flexible working around personal and family commitments.
  • Generous annual leave starts at 28 days and increases to 33 days after three years of service (pro rata for part-time). With an additional yearly leave buy policy.
  • A comprehensive Employee Assistance Programme (EAP) to support health and wellbeing.
  • Regular possibilities for growth through internal and external training sessions.
  • A fantastic office site near transport links and the redeveloped town centre.
  • Internal sports and social club opportunities.
  • Memberships to the leisure centre and gym are discounted.
  • Supermarket discounts.
  • An interest-free loan for a rail season ticket.
