Job Overview:
As a Mental Health Practitioner within our CREST-CEN Pathway, you will support individuals transitioning from Level 2 care to community settings. This includes coordinating care in alignment with the evolving national direction of the Care Programme Approach (CPA). You will provide evidence-based, trauma-informed interventions, addressing rehabilitation needs while managing risks in the community.
What You’ll Do:
- Work creatively with individuals who have spent extended periods in hospital, focusing on improving outcomes through innovative approaches.
- Support individuals with complex emotional needs, often referred to as personality disorders, as they transition to community-based care.
- Provide care within a 7-day service, with flexible hours of operation (Monday to Friday: 8 am – 8 pm; Weekends: 9 am – 5 pm).

Job Requirements:
- Visa Sponsorship: The Trust provides sponsorship for registered healthcare practitioners.
- Must have a valid driving license and access to a vehicle, as travel is required across West Yorkshire and occasionally to out-of-area hospitals.
- Knowledge of recovery principles and the ability to work in a way that promotes hope and therapeutic optimism.
- Ability to coordinate care and prioritize resources effectively.
Innovative Care Delivery:
As part of the CREST-CEN Pathway, you will assess individuals referred from case managers across the West Yorkshire region, identifying their rehabilitation needs and making recommendations to facilitate their successful transition into community living. You’ll work closely with a multidisciplinary team and other stakeholders to provide high-quality, person-centered care.
