Position highlights:

 

  • Position: Social Worker Care Navigator.
  • Location: Skokie Hospital.
  • Full-Time/Part-Time: 40 hours.
  • Hours: Monday-Friday Days: 8:00-4:30, rotational weekends—every third weekend

 

What You’ll Need:

  • Licence: LCSW or LSW is necessary.
  • Education: A master’s degree from a recognised social work school is necessary.
  • Certification: CPR and BLS are necessary.
  • Experience: Two years of post-graduate experience is preferred, with a focus on healthcare or mental health.
  • Experience with EMR platform is desired. Experience with Microsoft Office.

 

What you’ll Do:

  • Offers emotional support and counselling to patients and carers.
  • Helps patients comprehend their insurance coverage, as well as the post-acute level of treatment, and the necessary durable medical equipment for rehabilitation.
  • Coordinates daily between several departments, a multidisciplinary team, medical clinics, and community outreach to obtain patient information, ensure patient safety, smooth transitions of treatment, and monitor utilisation and total cost of care.
  • Responsible for outreach efforts to create and maintain excellent working relationships with patients and families, including members of the multidisciplinary team (physicians, office staff, diagnostic staff, nurses, social services personnel, home services, etc.).
  • Care coordination management of his or her patient group from the time of diagnosis prior to admission until 90 days after release.
  • Will be responsible for key success measures, which include reducing the total cost of care, length of stay optimisation, reduction in excess days, decrease in SNF utilisation, improvement in SNF care transitions, and reduction in 30-day readmission rate.
  • Completes and documents assessments in accordance with Endeavour and health plan standards on health plan portals.
  • Develops, implements, and updates an individualised treatment plan for each patient until the programme is completed.
