Job Description

Derbyshire Healthcare NHS Foundation Trust is seeking an enthusiastic and dedicated Lead Nurse to join the South Derbyshire and Dales Older Adult Community Mental Health Team (CMHT). This is a fantastic opportunity for someone with genuine interest and expertise in caring for older people experiencing these challenges.

A full UK driving licence and access to a vehicle for community travel are essential for this role.

Key Responsibilities

In this leadership position, you’ll take on a varied and rewarding caseload, including:

  • Conducting initial assessments and risk evaluations
  • Collaborating with service users to create personalized care plans
  • Acting as duty worker on a rota basis
  • Administering depot injections
  • Participating in multi-disciplinary CST (Care and Support Team) meetings
  • Building strong partnerships with colleagues across health and social care services

You’ll also play a vital role in team development by mentoring and supervising junior staff, as well as supporting and guiding student nurses during their placements.
