JOB DESCRIPTION

 

We are looking for a passionate and motivated Advanced Therapy Support Worker to help us care for patients in the community.

We are a responsive, dynamic, multidisciplinary community team that works with patients in their own homes, care homes, community support beds, and inpatient settings. Our service attempts to reduce hospital admissions, facilitate discharges, and provide rehabilitation for patients.

Our integrated team consists of occupational therapists, physiotherapists, advanced therapy support workers, and generic support workers who collaborate closely with our nursing teams and social services to give great care to our patients. This role is anticipated to provide cross-coverage support for our community’s rapid response treatment team as well as our Oakland Village rehabilitation beds.

The selected applicant will have generic therapy assessment and treatment abilities, be able to operate as part of a team, and have access to a trained therapist.

Excellent communication abilities are required.

There may be a requirement to work in triage, to assist the flow of referrals into the team, and to communicate with patients/caregivers/MDT members and referrers from a variety of agencies.

Access to the usage of transport will be required for travel between and across care settings.

The community therapy service is provided from 8 a.m. to 6:30 p.m. 7 days every week.

This would entail conducting initial assessments and reviews of patients, developing individual patient-led care plans, meeting therapeutic goals in accordance with competencies, and collaborating with other expert services within the Trust.

The position holder will be able to operate independently while adhering to Trust Lone Working rules.

To communicate with patients and caretakers about their health needs and treatment plans.

To report and take necessary action in response to any change in patient condition or scenario, as well as to communicate verbal and written information to all relevant members of the integrated care team.

To assist senior therapy staff with the ongoing development of services.

Maintain and record correct health records using System One/TPP in accordance with DCHS policies.

To exercise personal responsibility when using or supplying equipment, to ensure compliance with health and safety requirements, and to monitor and report to the relevant registered clinician on the effectiveness of equipment and aids to patients’ homes in order to make the best use of resources.

The post holder will manage and prioritise their own day-to-day work duties and activities, be able to operate independently and follow Trust Lone Working principles, delegate tasks to colleagues, and assist with administrative tasks for the day-to-day operation of the services.
