Detailed job description and primary responsibilities
1. Responsibilities.
Incident, Risk, and Investigations
Maintain professional conduct at all times and promote a favourable image of the Royal Free London NHS Foundation Trust in accordance with World Class Care values.
Access, monitor, and review clinical occurrences recorded through the Trust’s incident reporting system (DCIQ). Liaise with senior nursing teams and the clinical governance lead to ensure that investigations are completed and submitted within the Trust’s timeframes.
Collaborate with the Head of Quality Governance to oversee and manage all areas of the quality governance agenda, including the development of an effective patient safety framework that is quality-centric.
Ensure that the rolling programme of governance activities is delivered in accordance with the trust, hospital, division, and service needs.
Develop and coordinate effective integrated governance practices within the business unit.
Supporting the establishment and maintenance of good working relationships, communication, collaboration, and engagement with internal stakeholders.
Assist the Head of Quality Governance in evaluating and assessing the applicability of national policy and guidelines, and provide assistance to governance teams with implementation and evaluation.
Liaise with the Head of Quality Governance to agree on and complete deliverables as needed.
Provide expert assistance on quality assurance for patient safety and clinical governance.
Provide inspection and challenge to highly complicated, technical, and sensitive clinical and non-clinical data.
Analyse complicated data from incident investigations (including patient safety investigations) and audits, and collaborate with performance and contracting colleagues to develop suitable action plans to effect good change.
Leading and guaranteeing the division’s compliance with the Duty of Candour.
Collaborate with the complaints and legal teams to triangulate information from thematic evaluations of complaints, legal claims, and incident reports.
Provide monthly, quarterly, and annual reports that triangulate the information and data from clinical governance activities.
Leading the management of patient safety events and the dissemination of lessons from them. This includes reviewing and ensuring that investigations were carried out in accordance with agreed-upon guidelines and timeframes, determining whether the investigation is fair and/or requires additional information or interviews, and providing feedback on key areas for improvement, challenge, or further action. This will include:
i. Validating the SEIPS inquiry methodology, AAR, MDT, and subsequent action plans.
ii. Conducting thematic evaluations of patient safety events to discover trends and patterns among a certain group.
iii. Facilitating the spread of broader knowledge, exchanging best practices, and promoting quality improvement.
iv. Making sure that recommendations from patient safety events are included into quality improvement initiatives and plans.
Assist the Head of Quality Governance in ensuring that the Learning from Deaths policy is implemented on the hospital site, including a framework for regular thorough evaluations.
Encourage an open and transparent culture that responds quickly to prospective or real failures.
Support the management and coordination of hospital participation and the execution of recommendations from NCEPOD and NatSSIPs 2.
Conduct appraisals, monitor personnel compliance with mandatory and compulsory training, and keep records up to date.
Act as an excellent role model and mentor, encouraging greatness through practice.

Essential criteria
- Professional knowledge combined with additional specialist and managerial knowledge gained through training and practice leads to a postgraduate qualification or comparable experience in clinical governance.

- PSIRF approved training.
- A qualification in risk management.

Knowledge & Experience
Essential criteria
- Good understanding of risk and incident management in regard to the NHS Patient Safety Strategy.
- Knowledge of the requirements for the responsibility of candour.
- Working knowledge of PSIRF
- Evidence of working in an NHS setting.
- Evidence of Using Computer Packages
- Experience working with senior managers and leaders.
