East Sussex Healthcare NHS Trust is investing in Acute Therapy and Urgent Community Response services as part of a significant transformational initiative to establish a Home-First model that transfers care to the community, and we want you to be a part of it.
This extended service will operate across the integrated pathway, from acute to intermediate care services in the community, to provide patients with the treatment and rehabilitation they require while enhancing clinical outcomes and patient experience. We are investing in both therapy and care roles, and successful individuals joining our expanding service will have the opportunity to work with a dynamic, progressive, integrated, multiagency, and multidisciplinary team to provide fully integrated care to the people of East Sussex.
As a member of our team, you will collaborate closely with and be supported by our Professional Leads, Advanced Clinical Practitioners, Team Leads, Practice Educators, and the rest of the team, allowing you to fully develop your skills, training, education, and career – whether you are newly qualified or looking to advance to a more senior role.
A new post in East Sussex Healthcare NHS Trust’s Community Health and Integrated Care (CHIC) division will allow you to be a part of something genuinely innovative and remarkable.
