The Health Triangle PCN, located in Bracknell, Sandhurst, and Crowthorne in Berkshire, is actively seeking a PCN Clinical Pharmacist with a strong interest in diabetes care. We are a dynamic and growing network that prioritizes patient-centric care and professional development. Our multidisciplinary team (MDT) includes 4 Clinical Pharmacists, 5 Paramedics, 2 Physiotherapists, 2 Physician Associates, a Social Prescriber, and several Care Coordinators. Job Description:
We are looking for a dedicated PCN Clinical Pharmacist to join our team and focus on diabetes management and medicines optimization across our three general practices. This role is pivotal to our efforts in improving patient outcomes and ensuring the highest standards of pharmaceutical care.
Key Responsibilities:
- Conduct medication reviews, optimization, and reconciliation for patients, with a particular focus on diabetes management.
- Collaborate with GPs, nurses, community pharmacists, and other healthcare professionals to deliver comprehensive care.
- Manage chronic disease clinics, particularly diabetes, following local and national guidelines to enhance patient care and education.
- Provide clinical medication reviews, both in practice and in care homes, making informed recommendations for prescription and monitoring.
- Address common and minor ailments, offering support to patients in managing their medication and referring them to appropriate healthcare providers as necessary.
- Ensure seamless medicines management during patient transitions, such as hospital discharge, to reduce risks and improve patient safety.
- Participate in clinical audits, risk stratification, and implementation of local and national guidelines to optimize prescribing practices.
- Offer education and training to the primary healthcare team on medicines optimization and public health programs.
Qualifications:
- Degree in Pharmacy (MPharm/BPharm) and GPhC registration.
- Accredited in diabetes care or working towards it.
- Independent Prescriber (IP) qualification or working towards it.
- Strong understanding of general practice and medicines optimization.
- Full driving license.
Desired Experience:
- Member of the Royal Pharmaceutical Society (RPS).
- Clinical Diploma and minor ailments qualification.
- Experience with clinical systems such as Emis.
