Job Description

Join Our Team as a Family Health Worker

Are you driven by the desire to make a real difference in the lives of children and families? We’re looking for enthusiastic individuals to become part of our dynamic 0-19 Service at Leeds Community Healthcare NHS Trust. We offer flexible hours to suit your lifestyle, so if you’re ready for a rewarding challenge, this could be the perfect fit.

Our team pioneered the shift from Community Nursery Nurse to Family Health Worker, better capturing the evolving nature of our support for families throughout the city. In this position, you’ll play a key role in implementing the Healthy Child Programme, providing guidance and targeted help to kids and their loved ones. You’ll conduct thorough evaluations to uncover health needs, drawing on your expertise to address them effectively. A strong understanding of safeguarding is essential, along with the confidence to spot and handle any issues that arise. We emphasize collaboration, working hand-in-hand with multidisciplinary teams and external partners, so excellent communication—both spoken and unspoken—will help you tackle local health challenges head-on.

Teaming up with your 0-19 colleagues and community allies, you’ll showcase strong teamwork and interpersonal abilities to deliver prompt support. This includes helping families grasp important growth stages, encourage better living habits, and prepare for school. Your week might involve a mix of activities like: conducting child development check-ups in clinics, community centers, or right at home; leading breastfeeding workshops or offering personalized feeding advice; providing in-person or online assistance through our city-wide Baby Hubs; partnering with others to run the HENRY program; guiding parents with proven strategies on topics like potty training, healthy eating, behavior management, sleep routines, speech development, and general child growth; and connecting families to specialists with referrals when needed.

Ready to take the next step in your career with us? We promise an environment where your contributions truly matter.

If you’d like more details about the role, feel free to reach out to Rachel Watson, our Clinical Team Manager, at rachelwatson1@nhs.net or on 07944 160233.

Be sure to check out the attached Job Description and Person Specification for all the details on what we are looking for.

Note: We will close applications early if we hit our applicant limit, so don’t wait!
