Salary: £34,434 – £35,412 per annum (pro rata)

About the Role

Westmorland and Furness Council is seeking dedicated and compassionate Social Care Workers to join our Intermediate Care Team at Maudes Meadow in Kendal.

This service provides short-term residential support for adults who need assistance following hospital discharge or during a period of recovery. Through Intermediate Care and Discharge to Assess (D2A) beds, we help individuals regain confidence, rebuild skills, and return to independent living wherever possible.

Your work will play a vital role in supporting people at a key transition point in their lives, ensuring they receive the right care, at the right time, in the right way.

What You’ll Be Doing

As a Social Care Worker, you will:

  • Carry out strength-based assessments with individuals and their carers, focusing on abilities, goals, and independence
  • Develop and review person-centred care and support plans that promote recovery and reablement
  • Support therapeutic and reablement activities that help people rebuild daily living skills
  • Prioritise and manage assessments effectively, ensuring agreed timescales are met
  • Make clear, evidence-based recommendations for appropriate interventions
  • Work closely with health professionals, hospital teams, and wider social care services
  • Advocate for individuals’ rights and support informed decision-making
  • Seek guidance when needed and contribute positively as part of a multidisciplinary team

Travel Requirement

Due to the nature of the service, some service users may not be easily accessible via public transport. You must therefore be able to travel independently using a motorised vehicle.
